New Patients

Our Practice List is open to everyone living within our catchment.

Applicants who are eligible to receive NHS treatment may register at our practice.

To facilitate your registration please bring your NHS medical card if available with you.

 

In addition please understand that we are entitled to refuse non-urgent/non-emergency treatment pending your registration.

The Practice does not discriminate against race, gender, social class, age, religion, sexual orientation or appearance, disability or medical conditions.

As per NHS registration requirements, you will be registered with the Practice.  However, you do have a right to express a preference to see any doctor.

How to Register

To register as a new patient, please complete the NHS registration form for each individual. This can be done in one of three ways:

  1. Online: Complete the NHS online registration form. This is the quickest way to register. Click Here

  2. Download and Print: Download and print the registration form from our website. Click Here

  3. Visit Us: Collect a paper form from any of our surgeries.

Submitting Your Form

If you choose to register using a paper form, you can submit it by:

When submitting your registration, please include:

  • One form of photo identification (e.g., passport, driving licence).

  • One proof of address (e.g., bank statement, utility bill, or mobile phone bill) dated within the last 3 months.

If you do not have these documents, you can still register with us. Please call for more information.

Registering a Child or Young Person

For children or young people, a birth certificate is required to complete the registration process.

Next Steps

Once your registration form and documents are submitted, the registration process will begin. This involves:

  • Requesting your medical records from your previous GP surgery.

  • Processing the information, which may take up to 7 working days.

New Patient Check

After your registration is complete, you may be invited for a preliminary new patient health check to help us understand your medical history and health needs.

Welcome

We would like to welcome you to the practice so once we have received your records from your previous practice; we will offer you an appointment with the nurse for a new patient check.  This is designed to ensure your record is correct, give you some general advice on how the surgery works and she will also take some basic measurements.  Please bring any medications you are taking to that appointment.

 

If you need medical attention prior to this appointment please feel free to make an appointment to see the clinicians as required but be aware they may not have your complete file available.

 

We run an appointment system, you may book appointments in advance, however, if you feel you need to see a doctor on the same day, please ensure you ring at 8 am as we have a few appointments that become available every day for emergencies.

 

As per NHS registration requirements, you will be registered with the Practice.
